Community and District Nursing Specialist Practice

The Postgraduate Diploma in Community and District Nursing Specialist Practice is the NM C approved programme that prepares practitioners for the challenges of meeting complex care needs in the community in collaboration with key partners. Themes within the programme will develop highly professional, autonomous community nurses who are well prepared to support patients with acute, complex and palliative conditions in their own homes and other community settings. Key subject areas explored within the programme reflect the three core elements of the DH/NHS/QNI 2013 district nursing vision and model. These are: population and caseload management; support and care for patients who are unwell, recovering at home or at end-of-life; and care and support for independence. Theory will be applied in practice by students leading and managing person-centred care, which will focus on the individual and their specific needs.
